To save yourself time, you can download pdf versions of the forms here. If you bring the completed forms to the appointment, we will be able to start right away. If you don’t have access to a printer, I have the forms available at the clinic for you. Please note, the forms will take about 5 – 10 minutes to complete.

For best results, in the first appointment, your massage therapist will conduct a review of your health history and may perform postural and orthopedic tests to help determine the root of your problem. A personalized treatment will be developed from this information. To facilitate this, we recommend you book a one hour appointment for your first visit.
Health care should be client-centred and client-controlled. Each session begins with a discussion of the proposed treatment. You are free to refuse or ask for changes to the treatment. At any time, you can ask for changes or stop the treatment. You will be respected at all times in my care. You will be covered by sheets using various draping techniques throughout the entire treatment.
